The board information form provided here is self-explanatory, and is part of a proposal submission. The other forms are for use of agencies that have been awarded a grant, and need to request a payment or (for Special Project Grants) make a report (Reports and What We Want to Know) to Klee. All forms are interactive, allowing you to fill them out on your computer and then print. As you will note in the grant guidelines, a summary form is also needed for a proposal submission. This form also is interactive, but is available only through the Klee Foundation office, and only after an agency has been approved for proposal submission.
